Care Team Supervisor, Central Team
Permanent Full-Time
Hybrid Model (1-2 days in-office 2-3 days remote work)
What will be your role?
The Care Team Supervisor (CTS) is accountable for the overall operations and people management of their geographical team. This may involve onboarding, engagement, coaching, training (clinical and operational), career development, call/workflow monitoring, monitoring of performance indicators, absence management and performance improvement. The CTS’s team includes a mixture of unionized field staff (i.e., PSWs and PS Coaches) at the point of care and office-based staff (i.e., Care Team Scheduling Coordinators).
The CTS demonstrates leadership accountability within their region and supports other geographical teams as required. The role combines both clinical expertise and operational/systems proficiency to enable positive outcomes for clients and caregivers, service providers, office staff and funders. An important accountability of the role is to ensure that clients receive evidence-informed quality care and population specific quality outcomes, with a focus on client and PSW safety. The CTS ensures that the team adheres to VHA Home HealthCare policies and procedures, key performance indicators, contractual requirements, Ontario regulatory requirements, health and safety standards and CNO standards of practice. Ensuring fiscal accountability, monitoring resource utilization and resolving rejected billings is an important part of the role as well.
The CTS has a collaborative working relationship with VHA’s Care Team Clinical Lead. The CTS has supervisory oversight of the team of scheduling coordinators, clients within their geographical team and responds to escalated client or PSW issues that may involve joint visits with clients, families, funders.
An important part of this role is to provide communication leadership, which includes leading team meetings, acting as a change agent, quality improvement, union negotiations and de-escalation of urgent client or staff calls. Core business work hours for the Care Team Supervisor are Monday to Friday, from 8:30am-4:30pm. The CTS is also required to provide afterhours supervisory support on a rotational basis. Afterhours includes AM and PM support on weekdays and weekends (no over-night).
