York Region Looking for Registered Practical Nurse at Newmarket, ON Full Time

Job Description:
POSITION PURPOSE

Reporting to Director of Care, with day to day work direction from the Registered Nurse, practices as a member of the Multi-disciplinary Team, under the standards of the College of Nurses, Ministry of Health and Long Term Care, Facility Program Standards and the Centre. Participates and contributes to the assessment of individual residents’ needs and the development and evaluation of the plan of care.

MAJOR RESPONSIBILITIES

• Contributes to the development, implementation, and evaluation of individual care plans, through the Multi-disciplinary Care Team process.
• Assists in the orientation of the resident, family, Health Care Aide/Personal Support Worker and students.
• Assists with personal care as required.
• Maintains all resident’s records in compliance with the standards of the College of Nurses of Ontario and the policies of the Centre.
• Responsible for performing all duties in adherence to the guidelines and regulations of the Occupational Health and Safety Act, Long Term Care Act. and the policies and procedures of the Long Term Care and Seniors Branch and the Regional Municipality of York.
• Assumes responsibility for personal and vocational growth and development through participation in staff development programs, seminars and workshops.
• Participates in facility C.Q.I. and Occupational Health and Safety programs and on committees or work groups, as required.
• Liaises with residents and their family/visitors maintaining positive relationships with all stakeholders.
• Performs other related Duties as assigned.

QUALIFICATIONS

• Successful completion of a Practical Nurse Program from a recognized institution and a current Certificate of Registration from the College of Nurses of Ontario and practicum experience in a long term care facility or other geriatrics environment.
• Satisfactory Police Vulnerable Sector Checks (PVSC).
• Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.
• Computer literacy utilizing MS Office software applications and ability to research on the Internet.
• Good leadership skills and interpersonal and communication skills, including written, to work within a diverse community and ability to deal empathetically with residents, family members, etc., to contribute to their well-being.
• Ability to work effectively independently or as a part of a multidisciplinary health care team.
