Job Summary:
The Right at Home Canada Care Planner is the key liaison between the community and our Home Care Services. Our Care Planners function as the “Care Concierge” that match the family’s needs with a customized Care Plan and caregiving team.
The Care Planner is a crucial member of the Care Team. They are primarily accountable for all aspects of care delivery – handling care inquiries, service assessments, care planning, care team formation- all of which impact the overall client/family experience.
This is a hybrid position working predominantly remote, but there is also an in person requirement (training, client consults, team meetings, PPE days, community events, etc).
Status: Full Time
Salary: $55,000-$60,000 per year
