This Opportunity:

We are currently looking to fill one full time position for a Rehabilitation Worker in our Post Stroke Transitional Care Program (35 Hours per week) – Sault Ste. Marie, ON.

Roles and responsibilities:

• Implement individualized support services plans within the home and/or community for adults with a stroke.

• Provide services on a flexible basis during weekday business hours

• Reporting responsibilities including daily log note recording, develop service plans, and verbal reports as required

• Participate in training as required

• Follow policies and procedures as required

Qualifications and experience:

• Post Secondary diploma in Occupational Therapist Assistant and Physiotherapist Assistant program preferred or diploma in Human or Social Services

• Minimum of three years’ experience in Human Services field and/or working with people with a stroke

• Current CPR, First Aid is an asset

• Access to vehicle to use for business and valid driver’s license

• Proven consistency in work performance, including reliability and flexibility

• Excellent interpersonal communication and organizational skills

• Sound problem solving skills

• Proven ability to work independently / autonomously

• Bilingual
