Job Summary An exciting opportunity has arisen for Band 4 Full time Patient Flow Coordinator within the Emergency Department(Part of the UEC) The post holder is one of the key members in the team: help maintain flow across the planned and emergency care pathways. The role will involve continuing the good work we have started of collaborative multidisciplinary team working to ensure good communication from admission to discharge/ transfer.
Main duties of the job
- Maintaining the Cerner system in real time and maintain an accurate occupancy state in the Emergency Department, through face-to-face communication with the Nurse in Charge by ascertaining the current availability, known future availability and potential discharges.
- Liaise regularly with the Nurse in Charge of the SDEC & ED department regarding demand, acuity, likely pressures and present capacity and waiting time issues in the department, using internal escalation policies as required.
- Anticipate the demand on the Emergency Department in relation to Admitted and Non-Admitted patient flow and coordinate accordingly. Ensure all this actions are carried out in line with the agreed processes and policies.
- Establish accurate, reliable information to allocate and utilize capacity corporately (which can mean balancing conflicting pressures). This requires the post holder to do a significant amount of walking between different areas which can be physically demanding.
- Provide detailed reports on previous day’s performance and circulate them to ED GM, Clinical Lead and Head of Nursing

Job Description Job Responsibilities
- Support the Nurse in Charge and Consultant in Charge in the forward planning off each patient in the Emergency Department with particular emphasis on those patients approaching between 3-4 hours.
- Support patient flow through the emergency pathways the timeline principles of the 4-hour standard.
- Support immediate escalation of those patients approaching a 3 hour and the delivery of the 4 and 12 hours timelines and other department key performance indicators (KPIs) such as ambulance handovers, ambulatory,.
- Work in partnership with external colleagues to ensure the timely transfer of patients out of the Emergency Department.
- Monitor and maintain patient flow by means of Cerner (FirstNet & Capacity Management).
- Prompt clinical staff to ensure that discharge information and patient progress is recorded accurately in a timely fashion.
- Support the nursing body with the booking of transport for patients as requested.
- Gather and provide the necessary information, regarding blocks in flow for sharing at the Emergency Department huddles/Capacity board meetings.
- Submit to the General Manager/Service Manager key headlines (Out of Hours) explaining challenges and avoidance plans taken, at the end of the shift.
- Constantly check for patients and inform the relevant nursing/medical staff of any developments.
Person Specification Administration Experience Essential
- Demonstrable practical experience of working in an admin setting.
Desirable
- Previous experience with coordinating Patient flow within a care setting.
Skills Essential
- Good computer skills

- Knowledge of Cerner/FirstNet IT systems
