Ontario Health Teams (OHT) are being introduced to provide a new way of organizing and delivering care that is more connected to patients in their local communities. Under Ontario Health Teams, health care providers (including hospitals, doctors and home and community care providers) work as one coordinated team – no matter where they provide care.
The Burlington OHT is one of the 12 Initial OHTs to be progressed toward OHT designation as outlined in the Connecting Care Act, 20219. The Burlington OHT is seen as a leader in governance, system navigation, co-design and evaluation. As a new Not-For-Profit Incorporation, the Burlington OHT is in an exciting environment of growth and development as a new organization. PRIMARY PURPOSE
The Burlington OHT is looking for a detail-oriented and experienced Project Manager, ideally with experience in the health sector. The Project Manager will be responsible for managing a portfolio of OHT Integrated Care, as well as, Primary Care Sector-based projects from concept to completion. The Project Manager will work closely with the Burlington OHT cross-functional team, as well as the various BOHT committees to ensure collaborative input and integrated outcomes as the projects may require.
KEY RESPONSIBILITY AREAS
· Embeds in all OHT related projects the shared vision, guiding principles and strategy of the Burlington OHT, including principles of Health Equity, Diversity and Inclusion.
· Guided by the BOHT Strategy, including clinical and structural priorities, supports the implementation of projects and work plan deliverables for the BOHT.
· Project Manage assigned Primary Care sector projects related to prevention, early intervention and support for unattached patients.
· Develop project charters, work plans, project schedules, cost estimates and budgets as required
· Engage key stakeholders to ensure project activities are executed based on plans and standards, within the specific scope, budget and schedule
· Execute, track and monitor progress of the projects while distributing the appropriate project information to the appropriate decision-making structures of the BOHT
· Identify improvement opportunities and escalate key issues/risks; propose and execute solutions as approved that achieve quality work and process improvements
· Problem solve to result in innovative and creative solutions
· Complete project close out reports/lessons learned
· Develop and present material for review and information using excellent written, verbal and graphical communications skills with the ability to communicate technical aspects effectively
· Working with the BOHT Team, supports the measurement of OHT performance indicators to ensure regular measurement and monitoring of OHT initiatives.
· With the OHT Implementation Team, supports the implementation of data collection methods and analysis.
POSITION REQUIREMENTS
Qualifications
· At minimum, completion of a Bachelor’s degree program in Medical Science, Business or Health Administration, Health Informatics, Industrial engineering, Business or related field. Masters degree an asset.
· Project Management Profession (PMP) designation required
· Project planning, management and implementation experience required
· Change Management experience required
· Experience with Digital Health project management an asset
· Demonstrated knowledge and application of quality improvement principles (e.g. Lean Methodology) an asset
· Quality certifications (LEAN/Six Sigma/IDEAS) an asset
· Detail-Oriented
· Excellent written, oral Communication skills and Interpersonal Skills
· Excellent Time Management, meeting timelines
· Ability to work as a team in a professional and respectful manner that embraces diversity of thought
· Exercise good judgement, initiative, with ability to multi-task
· Experience in effective: Stakeholder Engagement; Relationship Management
Knowledge
· Knowledge of Ontario Health sector and the healthcare transformation to Ontario Health and Ontario Health Teams considered an asset

PHYSICAL REQUIREMENTS & WORKING CONDITIONS:
This position is mostly of a sedentary nature and requires long periods working on the computer. There are periods of stress when having to meet very tight deadlines.
The work environment is currently virtual (subject to change) with a minimum of 2 In-person meetings/month in the city of Burlington. In-person team meetings are mandatory and can be called at the discretion of the Executive Director.
Hours of work: Typical working hours are 8:30am to 4:30pm (Eastern Time), Monday to Friday, with some exception for community engagements that may occur virtually or in-person outside these hours.
For virtual work we require the individual to have a private, dedicated workspace from home free from distraction. A laptop, monitor and work phone will be provided.
