Position Title: BSO BEHAVIOURAL OUTREACH WORKER – Hamilton & Halton
Full-time Permanent (35 hours per week)
The Alzheimer Society of Brant, Haldimand Norfolk, Hamilton Halton (ASBHNHH) is looking for a Behavioural Outreach Worker to work in the Behavioural Supports Ontario (BSO) Community Team. The successful applicant will provide urgent and non-urgent in-home and transitional support for the BSO population with a focus on addressing responsive behaviours. The Behavioural Outreach worker follows and supports the client through episodic home visits to provide assessments, create and apply behavioural care plans, service coordination and collaboration with other providers (Hospitals, Primary Care, Home and Community Care Support Services, etc), and to educate the caregivers about strategies for reducing the escalation and/or prevention of responsive behaviours. Functions of practice for the BSO Community Team include:
· In-home holistic assessment
· Development of behavioural care plans
· Service coordination
· Education and coaching
· Person-centred care planning
· Advocacy
· Capacity building
· Overall system navigation.

The salary range for this role is $43,200 to $64,800 based on 1820 hours annually (35 hours per week).
Specialized Knowledge and Skill Requirements:
· Demonstrated and extensive knowledge and skill working with elderly individuals with various types of dementia, delirium, mental health, addictions, other neurological conditions and those who experience an acute behavioural change
· Demonstrated knowledge and best practice in behavioural assessment and case management
· Knowledge of current psychiatric and geriatric medications is considered an asset
· Knowledge of local Community Support Services, Primary Health Care and Mental Health services
· Knowledge of consent and capacity issues and legislation involving this population (e.g. role of Public Guardian and Trustee, SDM act, Health Care Consent Act, Mental Health Act)
Other Knowledge Qualifications
· Knowledge of client safety issues and demonstrated capacity to apply client safety practices
· Knowledge of PIECES framework and templates is a strong asset
· Experience working in interdisciplinary teams
· The ability to speak French is considered an asset
Skills

· Familiarity with standardized assessment tools (e.g. Folstein, Cornell, Cohen-Mansfield, MoCA)
· Ability to perform comprehensive bio-psychosocial assessment and build rapport with clients
· Demonstrated critical thinking skills to ensure preventative and early detection methods are applied

· Delivers care which recognizes both the uniqueness of each person and an awareness of one’s own contribution to that relationship, including personal attitudes, values and actions.
Communication
· Strong verbal and written communication skills

· Demonstrated troubleshooting skills and ability to problem-solve
· Ability in conflict resolution
Education and Facilitation
· Demonstrated educational/presentation and facilitating skills
· Sound understanding of principles of adult education
Other
· Proven ability to organize and prioritize work to meet deadlines
· Valid driver’s license and full time use of reliable vehicle, and willingness to travel is required (e.g. be mobile) and work in various settings/environments
· Strong computer knowledge and skills; familiarity with Excel is an asset
· Ability to work effectively with minimal supervision and as a team member
· Clear criminal reference check for the vulnerable sector
· Evening and weekend work may be required
· Some remote work may be required
Experience Requirements:
· A minimum of 3 years of experience in a comparable outreach case management or community outreach role working with the defined population.
