The Opportunity
Responsible for assisting clients and families with social and financial issues related to continuing care admissions or return to the community. Collaborating with the Community Care Coordinator, Transition Services to assist with the development of the discharge plan. Communicating with the client and family about progress toward the appropriate discharge location.
Program summary:
Geriatric Mental Health Program – a short-stay program for seniors who have primary psychiatric disorders who require assessment, treatment and rehabilitation to be able to return to the community. This is a voluntary program for those who are struggling with mental health conditions and who need more time for rehabilitation, improvement in their mental health and overall functioning.
