Detailed Overview: Under the general supervision of the Coordinator, Fraser South Eating Disorders Program (FSEDP), acts as a member of a multidisciplinary community mental health team, which provides services to youth and adults with severe and persistent eating disorders who require support in the community; provides a range of mental health services, including; lay counselling, support, and education to youth, adults and their families/supports; focuses on stabilizing, bridging and transferring the client(s) to and from appropriate services including tertiary services; provides escort services; works in close association with multidisciplinary community mental health staff, agencies and community providers; assists in the evaluation of process and outcomes to ensure the continuing quality of the service. Responsibilities:
Collaborates with the interdisciplinary team in interviews and assessments on clients and families; initiates, develops and implements the care plan for assigned clients, in collaboration with the interdisciplinary team; identifies actual or potential problems and strengths, plans interventions, implements and evaluates outcomes in conjunction with case manager from the Eating Disorder Program (EDP).
Communicates and consults with the Case Manager or the Eating Disorder Team about client care issues, resources and discharge planning; explains plan of care to clients and their families; teaches and demonstrates meal support with families; provides meal support to clients at home or in the community.
Provides client advocacy services with other agencies, services and service providers, including community agencies, schools, hospitals Adult Mental Health Services and MCFD Mental Health Services.
Supports client’s participation in community and other activities (as well as to attend appointments) through facilitating transportation.
Participates in planning and facilitating groups with the clients in the Eating Disorder Program, in conjunction with the Case Manager and the Coordinator.
Monitors clients’ progress; communicates and consults regarding the care and support plan with the client, case manager (EDP) and the eating disorder care team to ensure continuity of appropriate care to the client.
As appropriate, and as directed in the care management plan outlined by the Mental Health Therapist, the HCW will provide individual or group lay counselling, role modeling, social skill building and/or specialized consultation to caregivers and other involved professionals.
Assists with the development of an individualized developmentally appropriate psycho/social/vocational/educational/rehabilitation/treatment plan designed to overcome life skill deficits by assisting/teaching the client. This may include meals support, grocery shopping, menu planning and meal preparation, dining out.
Acts as a resource to the client’s family or the eating disorder care team, regarding the availability and suitability of support services to ensure the development and implementation of an appropriate care plan; facilitates the involvement of clients in self-help approaches and client initiated changes.
Assists in maintaining client records, in accordance with established standards, policies and procedures; writes reports of relevant observations including client and family teaching and evaluation of care to ensure continuity of communication and client documentation.
Maintains related reports, records, and statistical data, in accordance with established standards, policies and procedures.
Participates in the evaluation of client care programs, techniques, standards, policies and procedures by providing feedback about effectiveness and client care outcomes.
Participates in the orientation of students.
Participates in multidisciplinary conferences and with psychiatrist as required.
Performs other related duties as required.
Qualifications: Education and Experience
Grade twelve (12) plus successful completion of the Community Mental Health Worker Certificate and two (2) years� recent related experience or an equivalent combination of education, training and experience. Valid Class V BC Driver�s License and possession of a personal vehicle.
