Reporting to the Manager, Patient Services of the MAiD Team, this position is a full-time Registered Nurse Care Coordinator on the Home and Community Care Support Services team that serves the entirety of the Waterloo-Wellington geography as a patient advocate, educator, and support person for individuals inquiring or choosing the option of a medically assisted death.
The MAiD Care Coordinator will be supported to work to their full scope of practice to support this patient population, as required. The MAiD Care Coordinator will work collaboratively with a broad team across Home and Community Care Support Services Waterloo Wellington including Care Coordinators, community partners, hospitals, physicians and nurse practitioners, pharmacies, retirement homes, long term care facilities and other key stakeholders.
The MAiD Care Coordinator has advanced content matter knowledge and ensures patients are receiving timely access to care and support maximizing system capacity across all sectors. Interested candidates will have demonstrated a passion for the continuous growth and evolution of community based, integrated care. The MAiD Care Coordinator will participate and demonstrate an understanding of quality, risk and patient safety principles and practices, following all safe practices and procedures to support a safe patient and working environment.

This position may be required to provide relief at other Home and Community Care Support Services locations or teams as needed and may be part of the on call rotation as per the provisions of the ONA collective agreement.
