Home and Community Care Support Services Looking for Placement Coordinator at Mississauga, Ontario, Canada Full Time

The Placement Coordinator is responsible for the facilitation and authorization of long stay and short stay admissions to Long-Term Care Homes for applicants from the community, hospitals, other Long-Term Care Homes, out of the Mississauga Halton region and out of province.

What will you do?

• Respond to internal and external inquiries as required including information about placement legislation
• Manage, confirm, maintain and authorize applications for Long-Term Care Homes within the Mississauga Halton region and out of region/province applications
• Collaborate with client/family and internal and external partners to establish quality client outcomes in the placement process
• Maintain Long-Term Care Home waiting lists and vacancies
• Propose appropriate clients for bed vacancies at Long-Term Care Homes
• Acts as a resource to internal and external partners and committees to provide details of client placement application status and identification of issues, barriers and/or gaps
• Documents all aspects of the placement application process according to Ministry standards and Home and Community Care Support Services Mississauga Halton policies
• Compiles statistics for management use, as required

What do you need?

• Post-secondary education in health, social science (e.g. Psychology/Sociology) or social services field (e.g. Social Service Diploma)
• Minimum two (2) years recent work experience in related health or social services field
• Experience with long term care admission processes an asset
• Excellent knowledge of and adherence to relevant legislation
• Self-directed with the ability to organize, plan, prioritize and multi-task
• Ability to analyze information, problem-solve and make good decisions
• Detail-oriented
• Solid written documentation skills
• Ability to communicate effectively to ensure information transfer
• Strong customer service skills and conflict resolution skills
• Adept in the use of MS Office applications (e.g., Word, Excel, Outlook, PowerPoint, etc.)
• Ability to communicate in French or another language an asset.
