Homeless Health Peel is nurse-led service dedicated to delivering primary care to those experiencing homelessness or at risk of homelessness in the Region of Peel. Working within institutions and with social service partners, we strive to meet these individuals where they are to reduce barriers to accessing healthcare, and providing care where the individual is at, both physically and in their health journey
We are looking for a Nurse Practitioner to join us initially at our Stabilization Centre and eventually transitioning to primary care in the community when ready.
Qualifications and Experience
-Current registration and in good standing with the College of Nurses of Ontario as a Registered Nurse – Extended Class (RN-EC) with either Adult or Primary care specialty certificates, and no restrictions on practice.
– Candidates at all stages of their career are welcome to apply.
– Preference will be given to candidates who have worked with the homeless population or who have lived experience of homelessness.
– Preference will be given to candidates with mental health and addictions experience or who have lived experience of mental health and addictions.
Responsibilities
Clinical
-Provide primary care assessment and treatment to individuals across the lifespan to address acute, chronic, and specialty care needs. Diagnose and prescribe using the full scope of practice. Provide health education and counselling as required.
-Write orders to be carried out by the nursing staff. Provide on-call remote support to the nursing staff.
-Communicate with external partners as required (e.g. social services, pharmacies etc)
-Document according to established processes of the setting (e.g. EMR, paper orders) in a timely and organized manner in keeping with CNO standards.
-Provide documentation to patients as required (e.g. discharge letters, sick notes, forms)
Leadership
-Provide mentorship to nursing staff to support and develop their practice.
-Conduct chart audits to identify gaps in knowledge and care
-Identify and/or develop education activities to facilitate nursing practice.
-Develop and implement quality improvement initiatives to enhance care
-Participate in the hiring and training of new nursing staff
-Contribute to the development of the organization in keeping with its mission and vision.
Other
– Represent the organization in media and other promotional activities
