Responsibilities
Reporting to the Primary Health Care Director, the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives.
The incumbent works closely with Primary Health Care leadership, providers, teams and stakeholders to ensure localized support for planning, coordination, facilitation, and evaluation of PHC projects/initiatives. The Coordinator is responsible for developing and maintaining strong working relationships with health care staff, provincial and other Zonal and NSH departments, external individuals and partner agencies.
The incumbent will support the Northern Zone’s quality improvement initiatives for collaborative family practice and chronic disease management and wellness teams in primary health care.
The Primary Health Care Coordinator will work collaboratively with family physicians and other health care providers and teams to support change management strategies that are inherent in the services plan for primary health care.
Qualifications
Baccalaureate degree preferably from a health discipline/health related science
Current registration with professional association if applicable
Two years’ of recent related experience
Organizational and project management skills. Superior time management skills in order to prioritize and complete assignments
Chronic Disease Management experience preferred
Coordination and Project Management Skills
Broad knowledge of the health system with high level understanding of population health approaches, community development, primary health care, health prevention and promotion, and disease management
Experience in Practice Facilitation including coaching, facilitation, change management strategies, and quality improvement is an asset
Experience working in a family practice setting with family physicians an asset
Sound knowledge and skill working in an interdisciplinary setting
Ability to analyze information to identify priorities and emerging issues
Superior communication skills – written, oral, presentation
Efficient in Microsoft Word, Excel, PowerPoint, Outlook – smartsheets an asset
Demonstrated high degree of independent decision making, self-direction, motivation and team building/leadership skills
Understanding of Behavior Change Techniques
Valid Nova Scotia Driver’s License
Competencies in other languages an asset, French preferred.
