The Population Health and Value Based Health Systems portfolio at Ontario Health is seeking a one-year contract, full-time Manager for the Primary Care Program. This is an exciting opportunity to apply your healthcare knowledge and program and project management skills. The Primary Care Program works with internal and external partners to create clinical guidance and relevant data reports for the primary care sector and supports primary care engagement in the Ontario Health Teams strategy.
Reporting to the Senior Director, Primary Care and Social Determinants, the Manager will be responsible for program development, identify key initiatives and appropriate infrastructure, oversee the team and manage relationships with internal and external stakeholders. Since this is a new and evolving area, the Manager will be building policies, defining initiatives and identifying resources for a centralized primary care program within the population health and value-based health systems portfolio.
Once the fulsome program is established, the Manager will participate in strategic planning, coordinate execution of their team’s annual work plan, manage selected projects and services, and participate in program evaluation. They will play an important leadership role within this dynamic team and when collaborating with other teams across Ontario Health.
Here is what you will be doing:
- Support for the development of a centralized primary care program at OH – which includes mapping existing primary care activities across OH, making judgement calls on what should be centralized, defining work that should have a matrix reporting relationship, and identifying how to ensure alignment on decentralized primary care activities.
- Provide overall program coordination and program and project management leadership to a team, in which each team member is responsible for leading the planning, execution and delivery of projects related to primary care
- Participate in system- and regional-level performance measurement and management with an emphasis on using data for improvement
- Build relationships and partnerships with primary care stakeholders such as the Ministry of Health, OMA, AFHTO etc., so that OH can work with these partners to collectively address complex issues.
- Work closely with regions and external stakeholders on the implementation of primary care initiatives; define multiple implementation strategies to reflect the diverse and varied nature of primary care (i.e. family health teams, fee for service solo physicians, community health centres etc.).
- Work collaboratively with other Ontario Health programs to address common goals related to primary care
- Provide input into the use of data and analytics to improve quality at all levels of the system, using Ontario Health data holdings
- Work with teams across Ontario Health to create high value integrated data reports for primary care providers
- Recruit, supervise and inspire the team, including creating opportunities for growth and development
- Manage the intake of new projects, including assigning priority and resources
- Support senior leaders and executives in developing and clarifying stakeholder strategies and priorities
- Develop, implement, and report on the annual work plan, ensuring timelines and deliverables are met
- Work effectively with clinical experts and patient/caregiver representatives
- Manage the program budget
Here is what you will need to be successful:
Education and Experience:
- At minimum, completion of a master’s degree in a health-related field
- 7-10 years of progressive leadership experience, including 3 or more recent years focused on a role with similar levels of responsibility
- Clinical background is an asset
- Demonstrated experience with large projects of a complex nature
- Applied knowledge and experience with primary care
- Demonstrated ability to work collaboratively with diverse stakeholders, including senior management, clinicians, scientists, staff and external partners in a multi-disciplinary environment
- Demonstrated experience and skills with people leadership, including ability to lead through change and uncertainty
- Experience with quality improvement methodologies and innovative approaches to clinical engagement for change, including planning and implementation
- Experience managing and monitoring budgets

Employment Type: 1 year contract (12 months Maternity Leave)
