Trans Care BC works to support people to have the information they need to access gender- affirming health care and supports. Our services support access to care for those who identify as trans, Two-Spirit and gender diverse. We also provide information and resources to families, friends and loved ones as well as service providers.
Our Health Navigation team helps connect people in BC to gender-affirming health and wellness supports. Based out of Vancouver, the team is comprised of a small group of health navigators supported by a full-time clinical supervisor and part-time access to nurse and general practitioner for consultation as needed. The team works alongside service providers and community contacts from across the province to help people navigate to care and support as close to home as possible.
The Trans Care BC is seeking a Health Navigator to join their team. This Vancouver-based on-site role is integral to the direct client service and community capacity building efforts of the PHSA Trans Care BC program. Working from a client-centred, trauma-informed care model, the Health Navigator works alongside clients to support health care system navigation and coordinate information and service access. The team works in close relationship with the overall Trans Care BC Team, engaging community-based service providers and relevant medical staff across BC to support access to services as close to home as possible.
As the Health Navigator, you will:
Provide information on gender-affirming care options and pathways;
Engage clients from around the province by phone and email offering brief supportive conversation on a range of topics relevant to gender diversity;
Orient providers to training tools and resources available through Trans Care BC;
Offer community members and care providers information about gender-affirming surgical options and aftercare resources;
Maintain clinical electronic health record related to the care provided when relevant;
Where required and appropriate, support the coordination of aspects of care for clients deemed to be experiencing barriers to access;
Be involved in other initiatives of Trans Care BC as appropriate including being a part of info sessions about Trans Care BC’s resources, support resource development, community engagement, care provider training and community consultation initiatives
You have:
Bachelor’s Degree in a Social or Behavioural Science from a recognized program.
Two years recent experience in the provision of gender affirming care for trans and gender diverse clients.
You have demonstrated knowledge of:
Trans and gender diversity.
Indigenous and intersectional cultural safety.
Trauma-informed practice.
Strengths-based, client-centred practice, and a commitment to both.
Community development, community health agencies and services.
The range of service options available for the designated client population.
Solutions and strategies to reduce various barriers to care for the designated client population.
Facilitation, mediation and conflict resolution techniques.
Health inequities faced by designated client population.
Program development and community development.
You also have demonstrated ability to:
Work competently with the designated program/client population.
Establish and maintain supportive, trusting and professional relationships with clients and their allies.
Establish and maintain effective working relationships.
Prepare and maintain a variety of charts, records and reports.
Communicate effectively, both verbally and in writing.
Organize and prioritize workload and adapt to a changing community.
Plan and deliver in-service education sessions to lay and professional groups.
Perform a range of needs assessments with both individuals/groups.
Build partnerships among allied services providers
