The Gattuso Centre for Social Medicine seeks to address health inequities, working alongside community organizations, UHN clinicians, marginalized populations, persons with lived experience, patients, and all levels of policymaking to co-design, evaluate, and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN’s most medically and socially complex patients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity.
The Social Medicine Program is seeking a Program Coordinator to support high quality project development, implementation and evaluation by assisting with ongoing project coordination and evaluation as well as communication and dissemination activities.
Meeting Coordination: Organize and support executive and project-specific meetings, including logistics and minutes, as well as follow-up on completion of action items with team members.
Information Management: Maintain updated records on the status and activities of Social Medicine projects, partners, staff, and volunteers. This includes updating records and associated materials, updating descriptions of projects and collaborations.
Project Management / Coordination: Assisting in the ideation and execution of Social Medicine projects, including: developing work-back schedules, project protocols, research ethics submissions (as required), promoting timely completion of tasks by project team members
Program & Project Reporting: Assist in the completion, review and submission of report narratives and other reports as are required by donors and project partners.
Research Assistance: Availability to assist with research assistant tasks (transcription, preparing interview guides, conducting interviews/surveys, coding and analyzing, preparing reports)
Dissemination: Assisting in the planning, preparation, review, and production of dissemination materials (e.g., manuscripts, oral and multimedia presentations, web-based elements). This may include translation of content into plain-language for diverse audiences.
Finance & Fundraising: Coordinate the preparation and submission of program and research funding proposals and reports including: creating work-back schedules, promoting timely completion of tasks by team members, tracking financial elements.
Direct to the Program Manager, Social Medicine and Population Health Program, University Health Network
Manages multiple competing priorities, and able to rapidly shift focus in response to changing priorities
Lead discussion on project progress and present to the leadership team on key milestones
Implement best practices and obtain local client/patient, clinical, community, organizational and system perspectives
Collaborate with team members to ensure projects meet targeted and measurable outcomes
Lead all aspects of the project life cycle, working closely with the Program Manager and key stakeholders to:
Advance health equity in project delivery
Identify key stakeholders
Develop project metrics, surveys and data solutions
Ensure projects are completed within budget and specified timeline, and refine approaches as needed
Prepare reports and obtain stakeholder and project team feedback
Develop strong relationships with stakeholders and leverage multiple communication mechanisms and styles to effectively manage relationships with key stakeholders, ensuring engagement in, and continued support of, projects
Identify key issues and potential risks to project and stakeholder relationships. Provide recommendations to the Manager related to issue resolution options
Review data from various sources, analyze and interpret the information to inform project progress
Strong knowledge and understanding of population health, health equity, and the social determinants; as well as health system design and implementation to address inequities
At minimum, 5 years practical and related experience, and/or on the job training
Experience in program development and implementation
Strong experience building connections and relationships with community organizations
Strong knowledge of, and familiarity with, community resources to address the social determinants of health, including housing, poverty, and food insecurity
Dynamic knowledge of community engagement principles
Strong understanding of trauma-informed care and anti-racist, anti-discriminatory, anti-oppressive practices
Understanding of acute care setting and ability to navigate hospital systems an asset
Knowledge of the Ontario and Toronto health care and social services systems and public health an asset
Well-developed decision-making, problem-solving, and judgement skills
Excellent interpersonal, oral, and written communication skills
Well-developed public speaking abilities
Facilitation and/or teaching experience
Proven ability to work as a team member and independently
Ability to have and use diplomacy and tact at all times
Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code
