The Staff Pharmacist provides comprehensive pharmacy services to patients admitted to General Cardiology at Toronto General Hospital, with cross-coverage to other areas as required.
As a member of the patient care team, the principal responsibilities include:
Review and dispense all doctors’ medication orders for appropriateness
Identify, resolve and prevent drug therapy problems with the patient and health care team
Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines
Conduct medication reconciliation
Follow-up on medication incidents and promote safe medication practices on the unit
Respond to drug information questions
Patient education
Provide direct and/or functional supervision of technician staff
Facilitate and support clinical teaching and research activities within the department, while exemplifying UHN’s values
Possess a Bachelor’s of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent
Certificate of competency from the Pharmacy Examination Board of Canada
Active registration with the Ontario College of Pharmacists (Part A)
Hospital Pharmacy residency, an asset; hospital pharmacy practice experience with patient-centered care in cardiology and cardiovascular surgery
Excellent verbal and written communication skills
Excellent interpersonal skills
Excellent organizational and time management skills
Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
