Vancouver Island Inflammatory Bowel Disease Clinic Foundation Looking for Specialist outpatient clinic nurse at Victoria, BC Full Time

The Vancouver Island IBD Clinic Foundation nurse acts to provide longitudinal patient care, forming relationships with patients with inflammatory bowel disease (IBD). Inflammatory bowel disease is primarily made up of two conditions, Crohn’s disease and Ulcerative colitis. These conditions often affect young people, and are chronic conditions. Fortunately, our therapies are low risk and highly effective, making it a very rewarding area to practice in.

The majority of duties are performed from our office via telemedicine, with some occasional remote work possible, and some occasional in person assessments. The IBD nurse is expected to work-collaboratively as part of a team with physicians, investigators, research assistants, nurses, students, Island Health hospital and research staff, and administrative support staff.

We utilize a state-of-the-art electronic medical record (EMR) to deliver comprehensive patient care. Our EMR documentation system is one of the most sophisticated in Canada, making time spent efficient and high quality.

Patient Care Management

Duties

• Manage and coordinate the care of IBD inpatients, outpatients and research participants from across Vancouver Island requiring specialized monitoring and intervention as well as preventative care.
• Play a central role in coordinating interdisciplinary care of the most complex and vulnerable IBD patients
• Promote general and specific principles of health maintenance to patients and primary care providers
• Provide educational, emotional and social support and counselling to patients followed by IBD team
• Facilitate referral and engagement into various community supports and resources
• Identify educational needs of patients and their families, especially those with a new diagnosis of IBD or struggling with disability due to active disease or complications
• Assist patients in navigating health care system and various specialties
• Triage telephone calls from IBD patients, facilitating timely response and action
• Collaborate with IBD team and other IBD care providers nationally and internationally
• Attend monthly IBD Rounds and occasional evening continuing medical education (CME) to stay up to date
• Engage in continuous, supported education

Qualifications

Education & Experience

• Graduation from a recognized program for Registered or Practical Nurses or an equivalent combination of education, training and experience
• Current practicing registration as a Registered Nurse or Licensed Practical Nurse with the BC College of Nursing Professionals (BCCNP)
• Knowledge and competencies to work at full scope of practice as currently required by the BCCNP, including medication administration
• Additional degree or certificate in relevant discipline an asset

Compensation

VICF is a progressive employer offering salary based on years experience, some flexibility of hours, health spending accounts, and wellness accounts.

• Salary based on BCNU Wage grid plus 20%: LPN $34.12 – $41.24; RN $43.48 – $68.86 per hour
• Hours are 3-4 days per week based on clinic schedule
• Paid vacation of 2-4 weeks per year based on years of service
