The Upper Grand Family Health Team (UGFHT) offers the opportunity to join an innovative, community-based primary health care team serving the Arthur, Fergus, and Elora areas. We are looking for a talented team player to join us as we continue to provide a model of excellence in rural primary health care.
Position Summary
The Clinical Administrative Support is a new position that will support Upper Grand FHT clinical teams by streamlining intake for individual services and programs. This role and its responsibilities will develop over time. The successful candidate will report directly to the Executive Director.
Responsibilities
Support development of this new position including refining referral and intake process
Facilitate initial screening and provide other resources when necessary, including:
Assessing patient needs and available resources
Identifying situations that warrant fast-tracking or referral to external services, conducting risk assessments
Obtaining all relevant information from clients and booking initial appointments
Coordinating individual or group programming as assessed, and supporting access to other UGFHT services where possible or appropriate
Supporting clients’ navigation of other community and health services
Providing information to client’s regarding the services offered at the UGFHT
Support administrative assistant as the initial point of contact at the UGFHT for new requests/referrals for FHT services
Provide referral information for other community resources/services as appropriate to the client’s situation
Coordinate intake availability of clinicians and assign new cases as per agreed-upon processes
Provide administrative support and back up to the Administrative Assistant
Assist in quality and process improvement initiatives
Provide feedback in the development and evaluation of policies and procedures as requested
Provide input to management and clinical teams regarding patterns in client intake to support program development and enhancement
Document and maintain clear records of all patient interactions
Help identify service gaps for individuals or groups
Works closely with Manager, Quality and Programs
Other duties as required
Skills and Abilities
Time management abilities to effectively set priorities and complete tasks within agreed upon timeframes
Strong interpersonal skills
Ability to keep confidential all information regarding clients, patients, health care providers, employees, business operations and clinic/team functioning
Organizational ability to prioritize a wide range of job activities with accuracy and speed
Compassionate written and verbal communication skills which reflect the ability to work well in a team environment while maintaining respect and attentiveness to clients and fellow team members
Problem solving skills to identify and effectively solve problems and to help clients and team members solve problems
Ability to comfortably and respectfully raise issues with other team members and participate in resolving them
Ability to provide clear and timely communication with other healthcare and social service providers
Ability to deal with people in distress, de-escalation skills
Requirements
Successful completion of grade 12, plus completion of a minimum of one-year post-secondary program
Strong administrative skills
Minimum 3 years of relevant experience
Experience working with, and knowledge of the local health care system and community resources
Demonstrated ability to listen effectively and to build working relationships with clients, and with other team members
Proven ability to work in a collaborative team with a patient-centered approach
Excellent analytical, organization and decision making skills
Ability to work independently, self-directed and highly motivated
Ability to multi-task, prioritize and manage time effectively in a fast-paced environment
Flexibility to accept changes in job activities and responsibilities
Experience working in an EMR (e.g., Telus PSS) an asset
Valid Ontario Drivers’ License and access to own reliable vehicle required
Vulnerable Sector Check required
Proof of Covid-19 vaccination required
Hours of work will be 9:00am – 5:00pm
