Position Overview
Directly accountable to the Program Manager of the Day Hospital, the Nurse Case Manager has accountability for case management, using the nursing process to maximize the delivery of high quality care in accordance with the Centre’s Code of Conduct, policies and procedures and conforming to the College of Registered Nurses of Manitoba (CRNM) Standards of Nursing Care.
The Nurse Case Manager: initiates contact with the client, conducts initial assessment and standardized screening of clients; coordinates interdisciplinary care within Day Hospital including the arrangement of team conferences , lab tests and consultations; monitors health status and impact of team interventions; and liaises with other departments within Deer Lodge Centre and community agencies.
Performs all duties in accordance with established health and safety regulations/guidelines, policies and procedures (e.g. utilizing personal, protective equipment as per safe work procedures). Notifies management or their designates (i.e. supervisors) of all accidents, injuries, illnesses, safety or health concerns which are likely to harm themselves, their co-workers, or any others who enter the premises.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment.
