About The Opportunity
Reporting to the PHC Manager, Zone and/or Provincial and/or the PHC Director, the PHC-DFP Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management, and Department of Family Practice (DFP) portfolios. The PHC-DFP Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives assigned through leadership in each respective portfolio.
The scope of practice for the PHC Coordinator will include collaborative work with family physician’s offices and other health care providers to support change management strategies that are inherent in the services plan for primary health care. They are responsible for supporting the work of Primary Health Care and the Department of Family Practice through practice facilitation and practice support program coordination. They will work closely with Family Practices to facilitate and assist with the measurement and evaluation of practice improvements and to share/promote the successes of these improvements internal and external to NSH.

$34.11- $42.64 Hourly
