Addictions and Mental Health Services – Hastings Prince Edward Looking for Central Intake and Referral Coordinator at Belleville, ON Full Time

Position Summary:

Working as part of a multi-disciplinary team, the Intake and Referral Coordinator, under the general guidance of the Associate Director of Access & Transitions (or delegate), functions as the access or entry point for mental health and addictions services in Hastings Prince Edward through multiple methods, including telephone, e-referrals and scheduled open access. Central Intake and Waitlist staff receive and process all referrals, requests for service, information and/or consultations. Once processed, the incumbent conducts a central intake screening of the individual to determine care pathways and make referrals for services, liaising with other mental health and addictions professionals and other services providers for information gathering and service coordination on behalf of the individual. The Intake and Referral Coordinator will also provide resources and information to family members/caregivers about how to access services for their family members/friends and/or themselves. In addition, the Intake and Referral Coordinator provides waitlist management and interim support for AMHS-HPE as well as gathers and maintains statistics on receipt, unique characteristics and processing of requests for access. This position involves both in-office and outreach, completing central intake screening in the individual’s environment of choice, and community presentations. The incumbent is required to travel between AMHS-HPE locations as necessary.

Minimum Qualifications & Experience:

• University degree or college diploma in a related discipline;
• 5 years relevant/recent experience directly related to the provision of mental health or addictions services;
• Current Non-Violent Crisis Intervention (NVCI) Training, an asset;
• Applied Suicide Intervention Skills Training (ASIST), an asset;
• EMR/EMHWare experience, preferred;
• Demonstrated extensive knowledge of Hastings Prince Edward community and hospital programs and resources;
• In-depth knowledge of mental health/illness and substance use;
• Well-developed computer skills in the use of databases, word processing, spreadsheets, and communication applications; MS Office preferred;
• Exemplary verbal and written communication skills as necessary for effective collaboration of services and for community liaison responsibilities;
• Ability to maintain professionalism and communication effectively when there is emotional content;
• Well-developed mental health and/or addictions assessment skills;
• Demonstrated ability to prioritize needs with exemplary organization skills;
• Ability to deal with numerous competing demands in a fast-paced environment while observing deadlines;
• Must be able to work interdependently and in collaboration with the Central Intake Team to meet the objectives and deliverables of the program, as well as other AMHS-HPE staff members and community members;
• Thorough knowledge and understanding of community mental health service needs of clients and the availability of other community based mental health programs, agencies and resources;
• Well-developed presentation skills to present assessment information, provide effective information sessions to referral sources, and report on the team’s activities in various forums;
• Ability to travel between agency work sites and to community meetings;
• Required to provide a satisfactory Criminal Reference Check (CRC) and Vulnerable Sector prior to hire;
• Must possess a valid Ontario Driver’s License, have regular access to a reliable vehicle, and provide proof of adequate vehicle insurance, as necessary for the position
