We are seeking up to two full time, permanent Program Coordinator’s to join our Client Services team in Alberta and the Northwest Territories.
Position Description
Reporting to the Client Services Team Manager, the Program Coordinator is generally the first point of contact between the Society and our clients. This position is responsible for referring clients to services, delivering support and education programs, and building strong community and health care professional relationships.
This position is ideal for a highly motivated and self-directed person with a strong desire to serve in the field of human services, relating to the impact of Alzheimer’s disease and other dementias. Working virtually and in office, this position will assist with and facilitate programs and services across Alberta and the Northwest Territories. The Program Coordinator will:
·Promote and deliver AS AB/NT programs and services to clients utilizing various delivery methods like video and voice calls, emails, letters, or in-person.
·Facilitate and participate in the delivery of standardized programs for care partners, people living with early symptoms of dementia and the public.
·Provide outreach through the promotion of direct referrals to health and community care professionals.
·Coordinate, facilitate and oversee support groups for partners and caregivers and people with early symptoms of dementia; this may include working with volunteers & other personnel.
·Build a referral network through the development and maintenance of relationships with health care professionals and other local health service agencies through presentations, and information exchanges with informal and formal contacts.
·To maintain accurate, objective and current client and referral source records and reporting within a database.
·Network with partner organizations who also work in the field of dementia, mental health, disabilities, including community and care support.
· Where necessary liaise, seek advice and information from other parts of the organization to ensure information provided is accurate and up to date and based on accumulated knowledge.
·To assist in raising public awareness about dementia and the way it affects families, individuals and communities and to participate in events such as conferences and events.
·Collaborate as part of an information and support team, provide coverage for colleagues at agreed times or in unforeseen circumstances, as appropriate.
·Undertake other duties or projects commensurate with the nature of the position.
Qualifications
·Post‐Secondary education in Social Work, Health Services, Human Services or related field, or related experience in a health or social service agency, or education related function
·Familiar with services and resources needed by seniors and/or persons living with disabilities
·Experience in facilitation, adult education or community engagement is an asset
·An understanding of Alzheimer’s disease/dementia and caregiving issues is an asset
·Experience of record keeping, and report writing is an asset
·Strong verbal and written communication skills
·Demonstrated interpersonal skills and problem‐solving abilities
·Ability to remain professional and respectful in difficult and sensitive situations
·Ability to maintain confidentiality
·Ability to assess and evaluate client needs
·Empathetic and non-judgemental communication
·Commitment to and understanding of equal opportunities
·Proven ability to work independently as well as collaboratively with others as part of a team
· High level of proficiency in the Microsoft Office Suite
·Clear vulnerable sector check.
Working conditions
