Position Summary:
The Registered Practical Nurse, Ambulatory Liver Clinic delivers exceptional patient care through the application of the nursing process which includes assessment, planning, implementing and evaluating care delivered. The successful incumbent will collaborate with multidisciplinary health care professionals to provide care in accordance with hospital policies and best practice nursing standards. This is for a part time position; shifts are most often limited to days. The physical demands of the role may include but are not limited to sitting, standing, walking, pushing, pulling, lifting and bending. This position contributes to ensuring there is a safe environment for patients, staff and visitors. This role will:
Participate in the development of nursing care plans and counselling for clients with primarily liver disease, including patients with co-morbidities such as diabetes, renal, cardiovascular disease and mental illness
Use of a variety of teaching methods, incorporating principles of adult education to assist clients and their families members in gaining knowledge, skills and confidence in self-management of liver disease Regular monitoring of clients within scope of practice and evaluate the effectiveness of clients self-care plans through regular follow up visits with clients and /or families
Collaborate with client’s physician regarding clients treatment plan
Collaborate with the multidisciplinary team to build and evolve nursing standards for the Liver Health Program
Other duties as assigned.

Education and/or Professional Qualifications:
College Diploma, Registered Practical Nurse and valid certificate of registration from the College of Nurses of Ontario is required
Experience and Qualifications:
College of Nurses of Ontario Certificate of Registration in good standing, required
Minimum of 2 years’ clinical experience in an acute care setting required
Experience (recent within 5 years) with adult medical patients required
Experience and proficiency with liver and/or diabetes management is required
Current CPR certificate required
Medication administration required
Excellent physical assessment skills required
Must demonstrate familiarity with monitoring patients receiving blood products
Excellent communication (Verbal and written) problem-solving and negotiating skills required
Good working knowledge of diabetes and adult teaching principles an asset
Highly effective decision making, problem solving and organizational skills combined with interpersonal and communication skills, both oral and written required
Experience with Quality and Performance Improvement, collecting and using data to inform decisions, set targets and lead change
Reliable, flexible. creative professional able to work efficiently and collaboratively in a multidisciplinary team
Experience with conflict resolution
Demonstrated experience in performing telephone triage
Proven ability to foster good public relations and collaboration with community partners
Proven ability to work effectively in a changing dynamic environment with minimal supervision
Excellent interpersonal, verbal and written communication skills required
Demonstrated ability to develop an integrated plan of care and the ability to perform effectively as most responsible nurse in a total patient care model
Experience with difficult intravenous starts an asset
Ability to complete any other non- nursing responsibilities as assigned
Experience with medical directives an asset
Proficient computer and keyboarding skills, experience with electronic medical records. Meditech skills an asset
Experience with blood product administration & assisting with procedures and asset
Demonstrated ability to attend work on a regular basis is required
