In this leadership role the Case Manager is responsible for supporting admissions & discharges; coordinating the timely completion of the MDS Assessment Tool for a caseload of patients; ensuring continuity of care for a caseload of patients, which may include the provision of hands-on care such as ADL, transfers, bathing, wound management, management of IV & vascular access; managing patient & family concerns in a manner which supports patient focused care and problem-solving; promoting collaborative practice which includes coordinating care among team members; integrating the components of evidence based practice to improve the quality of patient care; contributing to program development including participation in Program CQI projects, managing resources in a fiscally responsible manner; and mentoring of nursing team members. The position reports directly to the Clinical Manager, Behavioural Health, for all functions pertaining to service operations and team functions on the assigned units.

Unit Summary
St. Peter’s Hospital Site of HHS is a Complex Care Hospital (Chronic Care). There are four inpatient specialty programs ranging is size from 44 beds to 63 beds, as follows: Behavioural Health, Medically Complex, Palliative Care and Restorative Care. There are also specialty outpatient clinics geared to the senior population offered through St. Peter’s Centre for Healthy Aging. All inpatient programs provide a full, on-site Interdisciplinary team, model of care to address the assessment, treatment and care needs of their complex populations. All programs aim to relieve suffering from significant illnesses with complicated symptoms, distress, loss of function and/or safety and behavior challenges, while promoting optimal functioning, enhanced quality of life and recovery, wherever possible. The potential for discharge planning to home or an alternate level of care is routinely assessed. Our teams strive for patient/family driven decision making and foster clinical excellence through the support of ongoing education and research activities.
