COMPENSATION: $52,535 – $63,260 Annually Prorated

To help fulfill its mission, LOFT is hiring a Psychogeriatric Case Manager.
JOB SUMMARY:
As a Psychogeriatric Case Manager you will be part of a Behaviour Support Team that provides direct care, education and support to seniors who are demonstrating responsive behaviours that may be associated with various types of dementia, delirium, mental health, addictions or other neurological conditions and who are ALC or at risk of becoming ALC in the acute care settings of the Ontario Health Central or Toronto Central area. You will assist in the creation and coordination of a behaviour support network, you will provide direct service to support the person served, family members and/or care-givers to mitigate escalating responsive behaviours. A key role of this position is to coordinate and support the transition of person served as they move to their next care destination.
Note: Occasional evening and weekend hours may be required. Frequent travel will be required using own vehicle.
