Role description:
Reporting to Director of Care, with day to day work direction from the Registered Nurse, practices as a member of the Multi-disciplinary Team, under the standards of the College of Nurses, Ministry of Health and Long Term Care, and our Home’s standards and policies. Participates and contributes to the assessment of individual residents’ needs and the development and evaluation of the plan of care.
- Successful completion of a Practical Nurse Program from a recognized institution and a current Certificate of Registration from the College of Nurses of Ontario and practicum experience in a long term care facility or other geriatrics environment.
- Satisfactory Police Vulnerable Sector Check.
- Knowledge and demonstrated ability in core competencies including customer focus, communication, collaboration and personal ownership.
- Computer literacy, previous PCC use an asset
- Good leadership skills and interpersonal and communication skills, including written, ability to deal empathetically with residents, family members, etc., to contribute to their well-being.
- Ability to work effectively independently or as a part of a multidisciplinary health care team.
- Ability to observe and accurately document resident condition and report same to the Registered Nurse in charge.
