CASE MANAGER

Full-Time Permanent

Location: Newmarket, Ontario

The Southlake Academic Family Health Team (SAFHT) is seeking a full-time Case Manager responsible for coordinating services from across the health system as well as other service systems (e.g. criminal justice, developmental services and addictions). While the position is expected to work primarily out of our Newmarket clinic, the opportunity for remote work does exist.

The Case Manager plays an integral role in the team based primary care of our patients through individual treatment/management, patient education/advocacy, and consultation/referrals to community agencies/services. Core responsibilities include hospital discharge care coordination, participation in our complex care program (this could include home visits), patient/caregiver navigation and will assist in the development of internal programs and services. The role does not include direct delivery of medical, clinical or other direct services. Opportunities for taking on leadership roles within the organization exist as per the need of the organization and the interest/qualifications and experience of the candidate.

Qualifications:

• Bachelor’s Degree in a related area of study or diploma in a related area of study plus 5 years comparable demonstrable experience.

• Current registration in good standing with the appropriate professional College of Ontario.
• Knowledge of community and health resources, preferably in the north York Region and south Simcoe County areas.
• Knowledge and skills in community based mental and behavioral health care and case management preferred.
• Experience working with persons with mental health issues, seniors and caregivers.
• Excellent written (including documentation) and verbal communication skills.
• Ability to work in a variety of settings with culturally-diverse families and communities.
• Advanced decision making and problem identification/solving skills
• Proven flexibility and ability to adapt to change.
• Proficiency in: Windows, Microsoft Office, Email, Internet, Electronic Medical Records.
