Position: Clinical Intake Coordinator
Site: Toronto Rehab, Rumsey Centre
Department: Cardiac Rehab
Reports to: Manager
Scale/Grade: D007
Hours: 37.5 hours per week 5 days/afternoons/evenings
Status: Permanent full-time
POSITION:
As an integral member of the Cardiac Rehab team, the responsibilities of the COORDINATOR, CLINICAL INTAKE include: utilizing clinical knowledge and skills to serve as an entry point for the timely access to appropriate clinical assessment and treatment services along the patient journey from referral to discharge; obtaining and maintaining current knowledge of internal/external services to communicate timely information; collaborating with representatives from all services to plan, develop, implement and evaluate a systems approach to client care; identifying gaps in service and participating in addressing such, internally and externally; playing a collaborative/facilitative role with respect to the management of resource utilization; identifies where improvements can be made, shares data regularly with management team to facilitate decision-making, perform cross-functional and/or other duties consistent with the job-classification, as assigned or requested (i.e., provides clinical consultations for patient populations served, coverage).
Assist care team with setting goals for quality assurance and best practices
QUALIFICATIONS:
At minimum, completion of a Bachelor’s degree in Health Sciences or recognized equivalent required
Master’s Degree preferred
At minimum 4 years practical and related experience
Registration/licensure as required by relevant professional body, e.g. College of Kinesiologists of Ontario, College of Physiotherapists of Ontario, College of Nurses of Ontario, College of Occupational Therapists of Ontario
Must have experience in direct clinical care with target population, including intake functions/triaging as well as in community relations and service development
Working knowledge of cardiovascular disease, assessment, management, prevention, rehabilitation and secondary prevention required
Ability to assess patients and match their needs with internal and external services
Excellent organizational skills
Proven ability to forecast and solve problems
Exercises initiative and good judgment with ability to multi-task
Produces high quality work while meeting deadlines in accordance to UHN standards
Proven ability to work independently and with self-direction
Proven ability to work effectively within an interdisciplinary team
Highly effective interpersonal and communication skills, proven ability to resolve conflict
Proven ability to self-reflect and act as a role model for respect, civility and inclusion in the workplace
Compliance with confidentiality requirements
Knowledge of applicable legislative, UHN and/or departmental policies
