This position assists with rapid re-housing for individuals experiencing homelessness or those at risk of homelessness. Client services include liaising with landlords including social housing, support in acquiring ID, and finding rental housing. The Community Health Facilitator provides short-term ongoing support to promote successful tenancies and individual well-being.
Position Summary
Under the direction of the Program Manager, the Community Health Facilitator is, in accordance with professional practice standards, responsible for the following:
Qualifications
Post-secondary degree or diploma in Health, Social Services, Community Development, or a related field

Communication

Experience in the use of electronic medical records
