Program Overview – First Step to Home, Community Care
First Step to Home (FSTH) is a transitional support program with housing for men 55 years and up who have been housing insecure or street involved, experiencing mental health and/or addictions issues, and geriatric challenges. First Step to Home provides transitional housing in furnished bachelor units as part of program participation. The First Step to Home team is comprised of health care professionals who work collaboratively, within a harm reduction and recovery-based framework to support participants to gain skills needed to make a successful transition to a more stable, healthier and independent lifestyle.
What You Will Do
Access client needs and develop and implement a plan of care or services with short- and long-term objectives, with ongoing monitoring until clients successfully graduate from the program.
Provide short-term intensive crisis case management services which includes assessment, supportive counselling, referrals to community resources.
Support the Manager with implementing the operational responsibilities of programs, services, and initiatives associated with the First Step to Home program and support services at 841 Queen St. E. site.
Provide daily oversight of program objectives and support with training of staff & student placements.
Work directly with the program participants to ensure they are meeting program expectations.
Provide support to staff regarding program initiatives, team building, and crises incidents through individual meetings with staff and participation in team meetings.
Support the promotion/awareness building of the programs and its services to the immediate and broader community.
Support the development of and implement program initiatives, such as food access, peer involvement, and social and recreational and life skills programming.
Through ongoing consultation with community agencies and development of a close collaborative partnership of all local providers, actively build a continuum of care for seniors with mental health and addiction issues.
Provide consultation to the team and other service providers on concurrent disorders and harm reduction, including offering programming on these topics for the participants of the program.
Support staff with the development and execution of Coordinated Care Plans and Case Plans for clients of First Step to Home and tenants at 841 Queen St. E. site.
Support in the development of common strategies to address tenancy and building issues.
Support WoodGreen Housing and Building Services with property management.
Supports the Housing Unit with tenancy issues at both 650 & 841 Queen St E sites, including pest control, hoarding, arrears, inspections, notices and work orders.
Support the Manager with the implementation of program/operational protocols.
Prepare statistical and program reports for the Manager as requested.
Ensure that the equity objectives and community development policies of the organization are followed within the programs.
Participate in regular supervisory sessions with the Manager.
Engage in training and education to upgrade skills as directed.
What You Bring to the Team
Bachelor’s of Social Work or Bachelor’s degree in a relevant field and equivalent experience.
3+ years of experience implementing programs; working with homeless and vulnerably housed individuals, and with older adults and seniors.
Knowledge of RTA and occupancy agreements, and property management.
Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption).
