What will you do?
Home and Community Care Support Services North East is seeking a dynamic individual to support the direction and delivery of Home and Community Care Services. Reporting directly to the Vice President, Patient Services, this position will plan, direct, and evaluate the delivery of these services and other specialty programs and services within the designated area. This position will be responsible for working with the Managers in monitoring hospital, community and system needs, service changes, patient satisfaction, caseload size and service utilization to ensure that programs respond to community needs.
KEY ACCOUNTABILITIES:
Contributes to the development, implementation and monitoring of Patient Services operational plan and priorities, and evaluates outcomes;
Participates in the development of service delivery performance standards and quality improvement processes, and ensures implementation within designated area(s) of responsibility;
Develops processes to monitor quality and identify and manage risks to patient care;
Supports managers and participates in the resolution of patient complaints and incidents;
Participates in the development, implementation, and evaluation of fiscal and operational planning;
Identifies budget requirements and manages approved funds in accordance with Home and Community Care Support Services North East priorities and needs;
Establishes partnerships and linkages with area partners, service providers and other stakeholders with the aim of improving the integration and utilization of health care resources along the service continuum, including Ontario Health Team development;
Provides ongoing leadership to service providers, managers and staff to ensure the delivery of quality patient care;
Represents the organization on both internal and external provincial, regional and local committees and working groups.
POSITION REQUIREMENTS:
Registered healthcare professional with a degree (or equivalent) in Nursing, a Therapy discipline, Health Sciences or other related field; Certified Health Executive considered an asset;
Significant leadership experience in directing patient service programs, using proven principles and best practices in direct care/service delivery models in hospital and community settings;
In-depth understanding of patient service delivery and North East area health care needs and services;
Good knowledge and understanding of health care systems and legislation governing home and community support services;
Excellent planning, organizing, critical thinking, decision making and evaluation skills;
Working knowledge of budget planning, resource allocation, financial monitoring, and reporting;
Knowledge and application of quality improvement principles and processes;
Working knowledge of Microsoft programs including Outlook, Teams, Word, Excel etc.
Excellent communications, interpersonal, and facilitation skills;
Effective coaching, negotiation and conflict resolution skills;
Must have valid driver’s license and access to a vehicle with travel required in the North East region as operationally needed;
Superior oral and written proficiency in English is essential, with French considered an asset.
