The Manager of Quality and Practice is responsible for enabling and supporting a proactive, integrated and just culture approach to patient safety and quality across the MSH Fertility Clinic and WIH Ambulatory Clinic. The main purpose of the role is to provide leadership and expertise in patient safety, quality outcomes and ensuring the strategic direction of the Magnet journey, Quality and Academic strategies.

The individual will report directly to the Senior Manger, Clinical Quality & Nursing Practice and will actively participate in the development of comprehensive quality and patient safety initiatives, identify/develop best practices for clinical excellence, service excellence and patient safety. The newly hired manager will also support the WIH Ambulatory Clinic located at University Ave and MSH Fertility Clinic in the appropriate use of resources to enable high performing/reliability systems. using resources within the portfolio to collaborate and enable high reliability systems. The successful incumbent will also be responsible to operationalize outcome measurement and reporting related to patient safety and quality practices; build capacity for quality and patient safety translating new knowledge and best practices and facilitating the objectives of the Ambulatory and Fertility Clinics.

Role And Responsibilities Include, But Are Not Limited To

• Identifying, Implementing and monitoring programs and activities related to quality, safety, risk and growth
• Supporting Interdisciplinary Quality Reviews and investigations; assisting with the completion of case review form which include root cause analysis, GEMBA observations, recommendations, goals, and validation of level of harm
• Supporting MSH Quality leads which may include monitoring of completion and sustainability of all recommendations
• Assisting with chart reviews as requested: including patient complaints and critical incidents
• Collaborating with the W & I program and Medical leads/Interdisciplinary teams for practice changes and product evaluation
• Supports the Academic Practice Strategy including enculturation of the Professional Practice Model in alignment with corporate strategy and operating plan priorities
• Liasing with the Clinical Educator(s) or Clinical Nurse Speicalist(s) to roll out changes and required education
• Supporting and monitoring Accreditation Canada/Magnet standards and requirements
• Supporting and monitoring Health Canada Standards
• Able to evaluate research and promoting the use of evidence based practice in professional standards, policies and procedures, clinical practice guidelines, quality indicators and the measurement of patient outcomes
• Monitors practice trends, current research, best practice guidelines, resource utilization and changing patient needs to make recommendations for skill or practice change based on current research, College of Nurses, SOGC, AWHONN, and CAPWHN information.
• Participates in program-based research with Interdisciplinary team and is aware of current research/quality initiatives within program
• Supporting Unit-Based Councils in alignment with transformational leadership principles and goals to engage frontline clinicians in decision-making
• Assisting in the roadmap to implement the Fundamentals of Care into nursing standards of care
• Participating in the strategic planning process to drive quality, safety and risk initiatives
• Participating in the current and future redesign of the Electronic Health Record, monitor implementation of required changes and associated KPI’s and CPOE changes
• In collaboration with the department of Infection Prevention and Control, and the program leads, monitoring that IPAC best practices and Perinatal standards are met in accordance with Sinai Health policies and procedures e.g. clean to dirty workflow, use of PPE, Hand Hygiene, Cleaning of equipment between patient use, clinical issues (e.g. ARO screening)
• Liaising with Medical leads, Interdisciplinary team, program lead and support departments for quality and risk issues e.g. pharmacy medication back orders, materials management supply issues, etc.
• Providing support to nursing leadership team related to the development of policies, procedures and medical directives
• Collaborating with unit leads and staff in the development and monitoring of the quality plan and unit specific quality metrics, annual unit scorecards
• Attending and participating in continuing education opportunities such as rounds, resident education, University of Toronto sessions, CAPWHN webinars
• Supporting the unit leads with the implementation of the approved quality plan and encourages staff participation in quality improvement/safety initiatives
• Supporting the unit lead in creating a culture of safety, risk mitigation, quality, and service to improve practices and service delivery
• Promoting and maintaining a safe patient care and work environment by assessing for and managing actual and potential risks; this includes engaging in safety audits, promoting staff accountability in workplace safety, and ensuring that adequate structures and processes are in place such as staffing, equipment, supplies, physical space, policies, procedures and best practices
• Providing support to the unit lead in addressing patient and family concerns and complaints. Encourages patient and family involvement in the complaint resolution process.
• Providing leadership in fostering and promoting a collaborative practice environment that is based on mutual respect among all team members
• Participating as a member/leader of departmental and corporate committees and teams
• Forming relationships and liase with key corporate staff
• Performing other duties related to the position as required

Job Requirements

• Master’s degree, or equivalent, in Quality and/or Patient Safety preferred. Those who have started in a program such as VAQS quality fellowship will be considered
• Bachelor of Science in Nursing, Masters of Science in Nursing an asset
• A minimum of 2 years leadership experience in a similar role preferred
• Experience in Obstetrics, Fertility, High Risk Pregnancies an asset
• Experience in Quality Improvement and Process improvement is an asset..
• Membership in professional organizations such as SOGC, RNAO, CNA an asset
• Proficient in Microsoft Office applications
• Proficient in EXCEL applications
• Demonstrated leadership in improvement science, patient safety principles, systems thinking, patient and family centered care, and project management
• Demonstrated collaboration with interdisciplinary teams involving quality improvement and continuing education
• Understanding of practice related to Electronic Medical Record, specifically CPOE (Computerized Order Entry) and PowerChart, Accuro and Feretility EMRs an asset
• Experience using the FMEA, and CPSI incident analysis framework
• External context and drivers for Sinai Health quality agenda: HQO, CPSI, Accreditation Canada, ISMP, PIDAC
• Knowledge of organizational structures and levers to achieve quality and patient safety outcomes
